Antenatal

Antenatal care

Antenatal care is the care you get from health professionals during your pregnancy. It’s sometimes called pregnancy care or maternity care.

You’ll be offered appointments with the midwife who works from the practice on Thursdays

Once the baby is born our team will be in touch to help you register your child to the GP surgery. If we do not hear from you within a week we will register the child on your behalf. 

Once your baby is 5 weeks old our team will be in touch to get you booked in for your post-natal care and baby check.

Accessing someone else’s information

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To request proxy access:

  • collect a proxy access form from reception during opening times

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accessing your record

Accessing your health record

Your health record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.

You can access your health record and nominate someone you trust to access it too.

You can use your NHS account (through the NHS website or NHS App) or SystmOnline to access your health record.

You’ll need to ask the surgery for online access to your full health record, or you’ll only see your medicines and allergies.

Request access to your full health record online

First, you need to register for online services and prove who you are. To request access:

  • fill out a register for online services form and choose to access your full health record
  • upload or visit the surgery with photo ID so we can confirm your identity
  • if you are setting up access for the first time, we will give you a username and password
  • go to SystmOnline or the app for access

Access to other medical records

For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.

Blood Test

You can also book via swiftqueue for blood testing. The first time you use this service you will need to register. You need to collect blood forms from the practice to take to your appointment.

Care Quality Commission ( CQC) rating

Care Quality Commission (CQC) rating

The Care Quality Commission (CQC) makes sure hospitals, care homes, dental and GP surgeries, and all other care services in England provide people with safe, effective, compassionate and high quality care, and encourages them to make improvements where possible.

They do this by inspecting services and publishing the results on their website.

You can use the results to help you make better decisions about the care you, or someone you care for, receives.

Cervical Smear

What is a cervical screen? 
All women aged 25 to 64 are invited by letter to cervical screening (a smear test) to check the health of their cervix. It’s not a test for cancer, it’s a test to help prevent cancer.

During the screening appointment, a small sample of cells will be taken from your cervix.

The sample is checked for certain types of human papillomavirus (HPV) that can cause changes to the cells of your cervix. These are called “high risk” types of HPV. If these types of cells are found, you will be invited back for further testing. If none are found, you will not be invited back till you are next due a test.

The nurse or doctor will tell you when you can expect your results letter.

When to book:
You will be sent a letter directly from NHS when it is your time to book, please contact us to book in when you receive this letter. Your appointment will be with one or our trained nurse team.

If you are a trans man with a cervix ( and not registered as a woman on the NHS system) you will not automatically be called as part of the NHS screening program, its still important to be screened so please contact us at any time and we will arrange for you.

Changing your contact details

Please contact us to let us know of any changes to your details.

Childhood Immunisations

Our team will contact you when it is time to book your child’s immunisations. Please see the NHS vaccine schedule. 

Once your child has had the 8-week immunisations the nurse will book them in for their following appointments for their 12 week and 16-week immunisations. If you have any questions, please don’t hesitate to contact us.

Contraception

We offer different forms of contraception for all our patients at the practice. We provide injections, implants, coils and the contraceptive pill.

If you wish to have an implant or coil, then you would need to contact the practice for a routine appointment with one of our GPs. They will discuss this with you further then add you to a waiting list, should these forms of contraception be suitable for you. Once on the waiting list a member of our non-clinical team will call you to book an appointment for this to be completed. 
If you require a removal or replacement, then you can contact the practice and ask them to add you to the waiting list (you do not require a GP appointment for this) and a non-clinical team member will call you to book this appointment.

If you wish to have the contraceptive injection you will need to contact the practice and book a routine appointment to discuss this with a GP. If you are suitable for the contraceptive injection, then the GP will advise you to book in with a member of our nursing team to get this administered. These injections are administered every 3 months.

For the contraceptive pill you will need to contact the practice and book a routine appointment to discuss this with a GP. If you are suitable for the contraceptive pill the GP will prescribe this for you. The contraceptive pill requires a yearly review with a nurse, or you may require a review every 6 months if your blood pressure fluctuates.

Ear Syringing

Our nursing team provide an ear syringing for our patients. You will need to contact the practice and book a 10-minute appointment for an initial review. The Nurse will assess you ear and gauge suitability and clinical need (look for excess wax) . They will the arrange a further 20 minute appointment for the procedure.  For the14 days leading up to this you will need to put olive oil in your ears twice a day, once, in the morning and once in the evening. With the blockage softened, the nurse will be able to syringe your ears with minimal issue.

Minor Injuries

Minor injuries

Hannage Brook Medical Centre are currently commissioned to provide a limited minor injury service for injuries sustained within the previous 48 hours that are relatively minor and can be dealt with at a GP practice, usually by our practice nurses, within our normal opening hours of 8am to 6:30pm, Monday to Friday.

Examples of minor injuries that can often be dealt with at the surgery are:

  • Lacerations capable of closure by simple techniques
  • Severe bruising requiring assessment
  • Recent injury beyond simple domestic first aid or where stitches may be required
  • Blows to the head where there has been no loss of consciousness

For anything more major the Whitworth minor injury unit or hospital A&E department would be more appropriate.

Minor Surgery

Some of our GPs are able to perform minor surgery within the practice. This includes:

  • Sebaceous cyst removal
  • Benign lump removal
  • Non cosmetic mole removals (interferes with day-to-day life) 
  • Non cosmetic removal of skin tags (interferes with day-to-day life)
  • Joint injections

If you feel you require this service , please make a routine appointment with a GP to discuss this further

Non NHS Services

Non-NHS Services

Some services provided are not covered under our contract with the NHS and therefore attract charges.

Examples include the following:

  • Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
  • Insurance claim forms
  • Prescriptions for taking medication abroad
  • Private sick notes
  • Vaccination certificates

The fees charged are based on the British Medical Association (BMA) suggested scales and our staff will be happy to advise you about them

Social Prescribing

Our Social Prescribers adopt a patient centered approach to help you have more control over your health and focus on what matters to you. They can link you to services in the community and support you with your needs which may include:

  • Social isolation and / or loneliness
  • Emotional wellbeing
  • Healthy lifestyle choices
  • Education and learning
  • Life changing events such as birth, retirement, divorce and bereavement
  • Housing, benefits, financial support and advice
  • Local groups and activities
  • Support with long term conditions management, mental health, social needs affecting wellbeing
  • Encouraging independence
  • Befriending, counselling and other support groups
  • Assistance to complete documents or forms

We are able to provide information and advice to those travelling abroad. Please contact our nursing team to discuss remembering to leave plenty of time ( we suggest 8 weeks) before you travel
