What is Social Prescribing?
Social prescribing is a way of connecting you with support in your community to help improve your health and wellbeing.
Sometimes, things like loneliness, stress, money worries, or housing issues can affect how we feel physically and emotionally. Social prescribing recognises that health is about more than just medical care.
Instead of a prescription for medication, we can help you access practical support, local services, and activities that make a real difference to your everyday life.
Social prescribing is part of the NHS approach to personalised care and is supported nationally by the National Academy for Social Prescribing.
How can it help?
We can support you with things such as:
• Feeling isolated or lonely
• Managing stress, anxiety, or low mood
• Staying active and improving your physical health
• Finding local groups, activities, or volunteering opportunities
• Getting help with finances, benefits, or housing
• Building confidence or getting back into work or routine
Everyone is different, so the support is tailored to what matters most to you.
What happens if I’m referred?
You’ll be offered an appointment (often by phone to begin with) with our Social Prescriber.
During the conversation, we will:
• Take time to get to know you
• Talk about what’s important to you
• Understand any challenges you’re facing
• Work together to find the right support
There’s no pressure—this is your conversation, and we’ll go at your pace.
Who is it for?
Social prescribing is for anyone who may benefit from extra support with their wellbeing. You might benefit if:
• You feel you need more support beyond medical care
• You’re feeling overwhelmed by day-to-day life
• You’d like help getting connected to your community
How do I access the service?
You can:
• Speak to a GP, nurse, or receptionist at the practice
• Ask to be referred to the Social Prescribing service
