If you live within our practice area and would like to register as a patient, please complete the registration forms available from reception, along with a new patient questionnaire, and return them as soon as possible to the surgery.
We would also ask you to bring along 2 forms of ID such as a passport/photo driving licence and a utility bill. ID is not necessary for children.
Upon receipt of your registration documents we may invite you to come into the surgery for a health check.
Once you are registered, you will have a named GP who is responsible for your overall care at the practice. Please contact the practice if you wish to know who your named GP is.If You have a preference to which GP is it is then we will make reasonable efforts to accomodate your request.
From 1st October 2014, as part of our government contract to provide medical services, we can choose to register patients that are not living within their practice boundary without the obligation to provide a home visit service to these patients.
The GP partners at Hannage Brook Medical Centre have considered this and have decided NOT to extend our practice list to include out of area patients. This will ensure that we are able to continue to care for the patients living within our current practice boundary without this level of care being compromised.
If you change your name, address, post code or telephone number, please give full details to reception. Remember to tell us if details for other members of your family have also changed.

A Patient Guide to GP Services
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient contact us on 01629 822434. (Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment.) You cannot register as a temporary patient at a practice in the town or area where you are already registered. We will take some details from you including name, address, date of birth, current GP details and NHS number (if known).
Please note that we will not register you as a temporary patient unless you need an appointment.
